CONGRESS MEMBERSHIP RULES
Important: by submitting registrations of congress member, individuals and groups automatically accept and agree with the congress membership rules.
WE DO NOT ALLOW NAME CHANGES
Pre-registration to the XLIV ERA-EDTA Congress has expired. Only onsite registration is possible, starting May 10 2008, at the Stockholm International Fairs and Congress Centre.

HOW TO SEND FORM AND PAYMENT
• Congress members can pre-register online or send/fax Pre-Registration form to:
ERA-EDTA Congress Office, Via Spolverini 2 , 43100 Parma, Italy, Fax: +39-0521-959242, e-mail: registrations@era-edta.org.
• Do not forget to send the Pre-Registration form completely filled in, when you send the payment by bank transfer: without it we cannot insert your name in the pre-registered congress members’ list and, therefore, you will not receive any confirmation or congress materials.
• In order to benefit of the Pre-Registration congress membership fees, forms and payments, all must be received within the deadlines. Submission of form without payment within deadlines, does not entitle to benefit of the reduced fees later.
• The mentioned deadlines are those of our receipt and not those of the postal stamp.
• Payment of fee and form must be submitted at the same time.
• Name of participant and/or company must be specified in payment description.
• If you send your Pre-Registration form by fax with a credit card payment, do not also mail a copy, since it may result in a duplicate registration and charged twice.

REQUEST OF PAYMENT: if you are in need of an official request of payment in order to transfer the congress membership fees, you must contact the ERA-EDTA Congress office well in advance before the set deadlines.
• Due to the fact that ERA-EDTA is a registered English Charity, invoices cannot be issued.
ERA-EDTA MEMBERSHIP
• The reduced fee is available only for ERA-EDTA members who have regularly paid the annual fees.
• If you wish to pre-register as member and you are not one, contact the ERA-EDTA Membership Office to become a member before pre-registering to the congress.

Confirmation letter and congress materials will not be sent to congress members with outstanding membership fees or status until they are settled.
CONFIRMATION AND DELIVERY OF CONGRESS MATERIALS
A confirmation letter and congress materials (badge, bag voucher and coffee tickets) will be sent by post to regularly pre-registered congress members/companies with no pending matters shortly after the set deadlines.
We strongly suggest to all congress members who have not received their badge in due time, to contact the Registration Dept (e-mail: registrations@era-edta.org or fax: +39-0521-959242) urgently. Replacement badges are subject to a fee of euro 10 each.
Congress bags can be picked up at the Bag desk located in the Registration Area, by handing in the bag voucher, starting May 10 at 8.00 hrs.
Please note that admission to the Registration Area will be strictly forbidden to all until May 10 at 8.00 hrs.
Group Registrations: all congress materials will be sent to the contact person of company directly and not to the congress members individually, third parties or branches. Upon confirmation of registration of a group, each sponsor will receive an identification code which will have to be used for correspondence and new registrations.
RECEIPTS/INVOICES
• Receipts are not automatically sent. If you wish to receive an official receipt of the payment, tick the receipt box in the form.
• Due to the fact that ERA-EDTA is a registered English Charity, invoices cannot be issued.
GROUP REGISTRATION WITH DEFERRED LIST
(Blank list)
(reserved to companies and institutions only)
Companies and institutions that wish to pre-register a group of more than 15 congress members may take advantage of the Blank List method. It is a procedure
with two separate stages for registering the whole group and for providing the individual names. All details can be obtained by contacting the ERA-EDTA Congress Office by e-mail registrations@era-edta.org or fax (+39-0521-959242). Deadline for bookings and payments is February 11, 2008.
After this date, only the ordinary pre-registration procedure is applied to all, groups included.
CANCELLATION POLICY
Before April 7, 2007 75% of the amount paid will be reimbursed
After April 7, 2007 no refund of the amount paid
All cancellations must be done in writing and must be received (not sent) within April 7 2008.
All refunds, if any, will be done after the Congress and will be in Euro currency only. In order to benefit of the refund, all congress materials must be returned to the ERA-EDTA Congress Office and must not be used for any purpose. If the registration must be cancelled for any reason (even at the last minute), it cannot be transferred or taken by another person.
ON-SITE CONGRESS MEMBERSHIP
(please read this carefully after April 7, 2008)
Each participant/company must fill in the on-site registration form in the computer with all details requested, at the New Registration desk (starting May 10, from 8.00 to 18.00 hours), located in the Registration Hall of the Stockholm International Fairs
and Congress Centre. Information such as name, full address, e-mail and fax will be mandatory for each registration, whether personal or sponsored.
It will be possible to pay by already issued bank cheques and bank transfers (in which case a copy of the original bank transfer receipt issued by the bank must be handed in while registering), cash (Euro only) and credit card (Visa and Eurocard/Mastercard
only).

Please note that after April 7, 2 008, you/your guests will not be included in the pre-registered file, even though the payment has been sent.
If you have paid by bank transfer the late fee after the final deadline of April 7, you must pay the difference towards the on-site fee and give in both receipts on-site in Stockholm. Or you must pay the difference on-site directly by means of one of the available methods of payment.
*Available only on-site: to qualify for this fee, you must be 35 years or younger and you must personally hand in a photocopy of your identification picture document,
show your original student’s card and give a signed original official letter from your mentor/university to proof your status. ERA-EDTA reserves the right to verify all declarations.
Companies or institutions will not be allowed to submit registrations at this fee.
**Exceptionally low congress membership fee (Euro 50) for students living in the country where the congress is held (Sweden), under the age of 28 years old.
This special fee is available only onsite: to qualify for this fee, you must be 28 years old or younger and you must personally hand in a photocopy of your picture ID (passport, driver’s licence, etc.), show your original Swedish student’s card and give a signed original official letter from your Swedish mentor/university to prove your status. ERA-EDTA reserves the right to verify all declarations.
Companies or institutions will not be allowed to submit registrations at this fee.
1 Day registrations cannot be purchased before, but only onsite starting May 10. This registration entitles admission to the session and poster halls only on the date mentioned on the congress badge and to receive the proper EACCME/UEMS Credits and Certificate of Attendance for that same day.
It is also possible to attend for two days by registering twice for one-day membership.
To avoid any kind of delay onsite, the ERA-EDTA encourages you to preregister for the congress in advance.
